GETTING STARTED

1. Save the file IL_WD365_10B_FirstLastName_1.docx as IL_WD365_10B_FirstLastName_2.docx

a. Edit the file name by changing “1” to “2”.

b. If you do not see the .docx file extension, do not type it. The file extension will be added for you automatically.

2. With the file IL_WD365_10B_FirstLastName_2.docx open, ensure that your first and last name is displayed in the footer.

a. If the footer does not display your name, delete the file and download a new copy.

PROJECT STEPS

1. You are in charge of processing applications from people interested in volunteering at Maple Ridge Hospital in Manchester, NH. You are developing a form that potential volunteers can use to apply to volunteer at one of three areas in the hospital. You will send the form via email, and plan to ask potential volunteers to fill in the form and then return it to you the same way. Change the height of the paper size to 7" so that users do not need to scroll when completing the form online.

2. Change the page color to Lime, Accent 3, Lighter 80% (7th column, 2nd row in the Theme Colors palette) to make the form more attractive.

3. Convert the tabbed text into a table and format it so that the form is easy to fill out:

a. Select the text from "Name" to "Years Known" and convert to a table consisting of four columns and five rows separated by tabs.

b. Add a new row above the row containing "Reference".

c. Merge the cells of the row containing "Reference" into one row and then center "Reference".

d. Add a new last row.

4. Split a cell and add new text to the table as follows:

a. Split the cell to the right of "Volunteered Before" into two columns and one row.

b. Enter No in the first new cell.

c. Enter Yes in the second new cell.

5. Add a content control for entering a first name as follows:

a. In the second cell of row 1 to the right of "Name", insert a Plain Text content control.

b. In Design mode, edit the placeholder text so that it appears as follows: Type your first and last name.

c. Format the new content control using the Emphasis style.

d. Lock the content control so that it cannot be deleted.

6. Add a content control for selecting a date as follows:

a. In the cell to the right of "Application Date", insert a Date Picker content control.

b. Select the date format similar to June 18, 2028.

c. In Design mode, edit the placeholder text so that it appears as follows: Enter the date of application.

7. Add a content control for selecting Availability as follows:

a. In the cell to the right of "Availability", insert a Drop-Down List content control.

b. Remove the "Choose an item." entry.

c. Add the following four choices for the drop-down list: Weekdays Weeknights Weekends No Preference

8. Add check box content controls so the user can indicate if they have previously volunteered at the hospital as follows:

a. Click after "No", press ENTER, and then insert a Check Box content control.

b. Change the properties of the check box to use No as the title and tag.

c. Copy the Check Box content control and paste it below "Yes". [Hint: Make sure no blank line appears below the pasted content control.]

d. Change the properties of the copied check box to use Yes as the title and tag.

9. Add a content control for specifying the area of interest as follows:

a. In the first cell of row 4 below "Area of Interest", insert a Plain Text content control.

b. In Design mode, edit the placeholder text so that it appears as follows: Enter your area of interest.

c. Lock the content control so that it cannot be deleted.

10. Add a content control for specifying Languages as follows:

a. Below the label "Languages" in row 4, insert a Combo Box content control.

b. Remove the "Choose an item." entry.

c. Add the following three choices for the combo box: English Only English and Spanish Other (please specify)

d. In Design mode, edit the placeholder text so that it appears as follows: Select an option or enter all languages spoken other than English and Spanish.

11. Add a content control for selecting the years of experience as follows:

a. In the cell below "Years Experience" in row 4, insert a Rich Text content control.

b. In Design mode, edit the placeholder text so that it appears as follows: Click here to enter the number of years of volunteer experience at a hospital.

c. Format the new content control using the Strong style.

d. Lock the content control so that it cannot be deleted.

12. Add a content control for indicating the Location as follows:

a. In the cell below "Location" in row 4, insert a Drop-Down List content control.

b. Do not allow the drop-down list content to be edited.

c. Remove the "Choose an item." entry.

d. Add the following three choices for the drop-down list: Acute Care Long Term Care Children & Maternity

13. Add content controls for entering the name of a reference as follows:

a. In the cell below "Name" in row 7, insert a Plain Text content control.

b. Copy the content control and paste it below "Phone", "Relationship", and "Years Known".

c. Turn off Design mode.

14. Insert a rectangle to separate the form from the page background as follows:

a. Draw a Rectangle: Rounded Corners around the table form.

b. Apply the Colored Outline–Green, Accent 1 shape style (column 2, row 1 in the Shape Styles).

c. Apply an Offset: Bottom Right shadow to the rectangle.

d. Send the rectangle to behind the text.

15. Restrict editing in the document to allow users only to fill in forms, and start enforcing protection without a password.

Your completed project should look like the Final Figures on the following pages. Save your changes, close the file(s), and then exit all Office applications. Follow the directions on the website to submit your completed project.
